I can't believe what I just did........
I was fishing in IF (while at work) and on the phone. So I was just raising my fishing level and being a robot with autoloot on. As I clicked on the fish button, I noticed the bind accept box....too late!
I let Old Ironjaw get away! AAAAAAHHHHHH
You aren't the first person to do that. I did that 5 or 6 times in STV for the rare fish...this while still needing my boots and line...
I feel your pain...did the same thing fishing for the Giant Sewer Rat...took me a week to get him again.
You could try using http://www.wowinterface.com/downloads/info11404-cAutoConfirmLoot.html to avoid it happening again.

Well, on a new character, I didn't let him get away.
123 casts in Forlorn Cavern, and I got him!
Maybe I will one day play the character that let him get away.....
On a positive note, the confirm box is going away for fishing in 3.1.
And I dislike the autoconfirmloot addon personally, I've almost looted stuff a couple times and hit no on the confirm box in dungeons.

Just caught Old Ironjaw twice on a new character....within 23 minutes of each other.........
Decided to level my fishing and cooking on a new warrior. Was going to fish to 150 and call it a night.
Started at 73 and caught him at 112....and again at 139!
Server time was 0130 and 0153.........
Go figure.
Update....
Decided to fish until I got it to 150, even after catching OIJ twice.....
At 0208 server time I caught him a third time!
I am going to get my lotto ticket as soon as I get off work!
